REGISTRATION GUIDELINES

Please contact Congress Secretariat for any kind of assistance or register onsite.

All participants are kindly recommended to register via the Online Registration Form. Please read the instructions on the website carefully.

Upon submitting the Registration Form, each participant will receive a Registration number by e-mail to be included in payment details (in case of payment by bank transfer). Please do not send any payments (bank transfers) without these details. Such payments cannot be identified

Registration Fees

Please note that unpaid pre-registration will not be considered as a valid registration unless fully paid and confirmed. Pre-registration summary therefore cannot be taken as a valid registration confirmation. Please make sure to pay your registration fee within the time frame for the selected registration type.

If your payment is not received before deadline date the later fee will be automatically charged.

All Congress fees are stated including the VAT.

* In order to qualify for the respective fee membership has to be valid until the end of July 2015.

** The official proof of student status should be submitted by email as a PDF attachment to the Congress Secretariat together with the registration.

Insurance

The registration fees do not include insurance of participants against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance.

All Participants need to re-confirm attendance of all functions in their Registration Form.

Confirmation

Upon receipt of the correct registration fee (clear of bank charges), each participant will receive a confirmation of registration. It is obligatory to bring this confirmation to the registration desk of the Congress venue as a proof of your registration otherwise we cannot identify your pre-registration if any problem occurs.
Please note that registration for the social events will be confirmed upon payment on a “first come, first served” basis and numbers may be restricted.

Payment

All fees for the Congress registration should be paid in EUR (€), free of all bank charges!

By bank transfer

Clear of any bank charges, clearly stating the participant’s name and Registration number.

Bank transfer can be made only upon receiving detailed information (account number, Registration number etc.) from the Congress Secretariat. Payments received without pre-registration details cannot be identified.

After 15 June, 2015 you are kindly requested to make all payments by payment card exclusively or to register on-site. Payment by bank transfer will no longer be available from this date.

Online payment by Card (Visa, Eurocard/Mastercard)

In case of rejected on-line credit/debit card payment, the registration will not be processed by the Congress Secretariat. It is recommended that the participant contacts credit/debit card issuer/bank first and checks if the on-line transactions are not disabled on his card. Afterwards, the online registration process can be repeated.

Credit Card Acceptance Policy

Please kindly note that for the purposes of the online registration and payment, only internationally recognized credit cards using highest standards of security are accepted. We accept following credit cards for the online payments: MasterCard, Maestro, VISA, VISA Electron and V Pay. For your easier reference, please check if your credit card is embossed, has 16 digit number and 3 digit CVC code at the backside plus a magnetic slip. Should you have any question in regards of validity of your credit card for online registration, or should you wish to check if payments with other cards like American Express are possible, please feel free to contact Congress secretariat.

Final invoice

The final invoice will be provided to the participants only upon request, the Secretariat will not send this document automatically. Please contact the Congress Secretariat to ask for your receipt.

Cancellation / Refund Policy

Cancellation of Registration:

  • Until 30 April 2015 – 50 EUR administrative fee will be deducted from the refund
  • From 1 May 2015 till 15 June 2015 - reimbursement of 50% of the fee will be paid
  • From 16 June 2012 - no refund can be processed under any circumstances

Cancellation of Extras:

  • Until 16 June 2015 – 20 EUR administrative fees will be deducted from all refunds
  • From 17 June 2015 - no refunds can be processed under any circumstances

All cancellations must be notified in writing (by mail, fax or e-mail) to the Congress Secretariat according to the following conditions:

  • Complaints submitted after 31 August 2015 will not be considered.
  • Cancellation of registration fee - refer to the above conditions.
  • Transfer of registration to another person - a fee of EUR 20 will be charged.
  • All registrations received by 22 July 2015 will be included in the list of participants as the online registration is being closed

I agree to the collection, preservation and processing of personal data contained in this form, by the administrator, C-IN, Prague Congress Centre, 5. května 65, Prague 4, 140 21 (or its employees) for the purpose stated below.
This consent is granted for all of the information contained in this form, and throughout the five years from the date of consent. At the same time I am aware of / and of their rights under § 12 and 21 of Act No. 101/2000 Coll. on the protection of personal data.
I am familiar with all completed parts of this form, and I declare that all information are accurate and truthful and are provided voluntarily.

Purpose of Processing Personal Data

In accordance with § 5 of Act No. 101/2000 Coll. on the protection of personal data, all information given on this form are collected and processed solely for purposes of processing registration and accommodation requirements for the ESB 2015 and for sending related messages to the data subject via electronic means pursuant to Act No. 480/2004 Coll. Summarized data from this form may be used by the administrator for internal statistical purposes and for internal use of the administrator.

Statement by Administrator

Data administrator declares that they will collect personal data to the extent necessary to accomplish the intended purpose and may be processed only in accordance with the purpose for which they were collected. Employees of the administrator or other individuals who process personal data under contract with the administrator and other persons are obliged to maintain the confidentiality of personal data, even after termination of employment or work.

Responsibility

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the Congress be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes.
